Life Chances

The Life Chances VCSE Reference Group (covering Devon including Torbay and Plymouth) has been formed to contribute to the achievement of Life Chances project outcomes which aims to develop Social Prescribing opportunities across Devon.

This group reports to the Life Chances Steering Group, promoting collaboration and identifying strategic opportunities to spread and sustain success and learning in relation to Social Prescribing across Devon. This group does not attempt to be ‘representative’ of the sector as a whole, but provides the VCSE sector with a stronger, more influential voice and provide a useful sounding board.

So what is Social Prescribing? 

"Social prescribing involves helping patients to improve their health, wellbeing and social welfare by connecting them to community services which might be run by the council or a local charity. For example, signposting people who have been diagnosed with dementia to local dementia support groups."
